The only outfitter operating hot air ballooning trips in Peru is the U.S. owned company Globos de los Andes who have offices in Cusco:

They offer balloon flights (although on a rather inconsistent basis) mainly in the Sacred Valley, near Urubamba. Flights are mainly during the dry season (May to September). Prices range from US$60 for a tethered flight over the Moray Inca ruin to US$700 per person for an overnight stay in a local bed and breakfast with meals, transfers and a stunning morning flight over the Valley. Contact them in advance for further details and reservations. They do not offer flights over Machu Picchu.
